If you are a private contractor requesting a permit to work on City trees, you will be required to provide the following:
To obtain approval from the City, you must submit the below Permit Application prior to performing any work on a City street tree. You will be notified of approval in writing.
Required Insurance for City of Modesto vendors. You are required to upload this information into our Insurance tracking system (PINS) prior to the issuance of this permit.
In order to apply for this permit, you must have an approved application to be a vendor on file. You must be listed as an Approved Tree Vendor and have current Insurance documents uploaded to our PINS system.
Approved Vendor shall obtain an encroachment permit.
Approved Vendor is responsible for removing all debris from job site. Debris may not be left in the street, public right-of-way, sidewalk, or alley. Small or fine debris material shall be cleaned up and or placed in a manner as to keep it from entering City storm drains.
If a Notice was issued to the Customer by the City, a copy of the Notice must be submitted with this Permit Application.
A copy of the estimate for the work to be performed must be submitted with the permit.
Permittee hereby agrees to indemnify and hold harmless the City of Modesto, the City Council, any other officer, employee, or agent of said City, from all suits, claims or actions of every kind brought as a result of any injury or damage suffered, whether to persons or property, which was sustained as the result of any act or omission of the Permittee, their employees, contractors, agents, or anyone else acting on behalf of Permittee, in the maintenance of any public street tree under the terms of this permit.
