Program Announcement

Applying for Small Business Assistance Program Benefits

All applications are done online, using the buttons for "New Application" and "Saved Forms" at the bottom of this page.

Completing and Submitting an Application

When you click on the "New Application" or "Saved Forms" buttons below, you will be taken to the FormCenter to login to or set up your profile if you are a new user.  Log in to your profile and then work on and complete the application online.  As you progress through the form, be sure to click on the "Save Progress" button in the form’s upper right corner.  If you need to pause and come back to your application, open this form from your Saved Forms in your account.  

While working on the application, you may use the email and phone contacts in the right column of this page for questions regarding each Small Business Assistance Program component.  Due to volume of applications, please allow 48 hours for staff to respond to your question.

When you submit your application you can have a copy sent to your email address and/or printed.  

After You Have Submitted Your Small Business Assistance Program Application

You will receive a confirmation message that your application has been received.  Your application will be reviewed by the Departments and Divisions responsible for each Small Business Assistance Program component.   Please allow seven to ten business days to evaluate your application and reply to you.  You will be contacted by email for next steps.

Before being approved for any benefits, you may be required to provide documentary proof of qualification requirements.
